What is causing my HF roll off?

just a quick question though.. When you are doing these measurments you are sure you are using exclusive mode on both the mic and preamp right?

Yeah, everything is in exclusive mode

Are your measurements fixed point or is this moving method? Initial glace, to me, would indicate that the tweeters aren't lined up with the mic and/or the speakers are not completely equidistant from the fixed point.
