Reverse image search resulted in something about that former US president with the orange skintone and impeachments. Probably would be labeled 'politics' and removed.
The original South Park scene is about mr Burns having all deceases known to man and some new ones, which keep eachother in balance because they don't all 'fit through the door' at the same time. Like the three Stooges.

I found that caption, it is not the one I remember. . . the one I am thinking of it referred to a particular mayor's scandal/decisions.
I think Rudy Giuliani or Bill De blasio I cannot remember exactly. . .
Maybe it was a inside joke for local's.

yeah, I knew that.

In fact, I have a mug from an old (much beloved, by me at any rate), now defunct analytical instrumation company, ISCO, with a space-filling (CPK) model of caffeine on it. The perfect mug for a biochemist.
